To fund the transitional staffing necessary to create a new hybrid model of service that will meet the needs and desires of their intellectually and developmentally challenged Sonoma Valley clients in the continuing pandemic. Amount: $20,000, 8/20/21

Health Justice
Orgainzation: The Hanna Institute
This initial summer program aims to engage in a collective generation of knowledge of what self and community care are, and to practice and disseminate healing strategies to the youth of Sonoma Valley, specifically high school students. Students will learn: (a) to create risk assessment, safety planning charts, and follow up plans, (b) options for wellness, healing, and support, (c) strategies to access joy and healing centered engagement, (d) support in dissemination of the strategies among peers and within community.

Vaccination Navigation Support for Seniors
Organization: Vintage House Sonoma
This initiative provides Sonoma Valley seniors with assistance to register for and make vaccine appointments through the hiring of a part-time, bilingual vaccine navigator at Vintage House. Vintage House is on the front lines of tracking the various vaccination venues and helping seniors overcome technological and language challenges, so they can make appointments and get vaccinated. Meanwhile, Vintage House's vaccinated volunteer drivers are ready to help drive seniors to their appointments if needed.

Sonoma Valley Mass Vaccination Support
Organization: Sonoma Valley Hospital Foundation
This initiative enables Sonoma Valley Hospital (SVH) to begin planning and executing public mass vaccination drive-thru clinics, in association with Sonoma Valley Fire and Rescue Authority (SVFRA) and the Sonoma Valley Community Health Center (SVCHC), and the City of Sonoma. The funds granted to the Sonoma Valley Hospital Foundation will enable SVH to hire a Vaccine Clinic Coordinator, who will be responsible for overseeing the scheduling platform for the public, clinic volunteer schedules, the paperwork for registration, consent, and reporting, as well the communication between community partners.

Update: Vaccination Clinics at the Sonoma Valley High School was a historic success. Over the 6 weekends of clinics they fully vaccinated about 3,500 people with 2 doses through the support of their partnerships and 45 dedicated volunteers.Partners include: Sonoma Valley Community Health Center, City of Sonoma, Sonoma Valley Hospital, Gurnick Academy, Marin Health, Sonoma Valley Fire District, Schell-Vista Fire Protection District, Sonoma Valley Rotary, Sonoma Valley Unified School District, Sonoma Valley Catalyst Fund, and the many Community Volunteers! At the end of March the hospital will take a step back and decide whether or not to continue leading these clinics or support other local vaccination efforts.

Improving Vaccination Acceptance
Organization: Sonoma Valley Community Health Center
This initiative supports the Sonoma Valley Community Health Center's Vaccine Education and Outreach program by enabling 1) the hiring of a Marketing Project manager; 2) the sponsorship of Charla Communitaria and other Spanish outreach efforts; and 3) the creation and placement of effective advertisements for the targeted audiences to address vaccination concerns. The educational outreach program is an essential part of promoting the vaccination of 27,000 or +80% of our valley population for herd immunity.

Update: Two Charla Comunitarias (community conversations) on vaccinations have been sponsored. The position was filled in early March and is now organizing the semi-monthly vaccination outreach coordination meetings.
